COMO AYUDAR A REALIZAR INTERVENCIONES QUIRUGICAS EN AREAS RURALES ALEJADAS DE NUESTRO PAIS

En noviembre del 2022 estuvimos en una misión humanitaria en el municipio de la Macarena apoyados por el grupo Omega de nuestras fuerzas militares. Sin ellos no hubiéramos podido tener el conocimiento que hoy tenemos de las necesidades de ese municipio, ya que viajar allá no es fácil…
Nos desplazamos con nuestro grupo de profesionales con un objetivo primordial. Conocer la población, preguntarle a cada persona por sus enfermedades, por sus dificultades, por sus anhelos, por su vida misma y así motivar a nuestro corazón acerca de cuál podría ser un proyecto de apoyo a ese municipio. Logramos el objetivo porque conocimos personas que vinieron a buscarnos desde sitios muy distantes, atendimos más de 400 personas y si bien es cierto solucionamos muchas cosas también nos quedó el sabor amargo de no poder resolver otras más. Y cuando el presidente de las juntas de acción comunal, al final de la jornada, se presentó y nos ofreció el apoyo de las 350 juntas de acción comunal de la zona de la Macarena terminamos aún más conmovidos para cooperar con estas personas. Terminamos la jornada y con la felicidad plasmada en todas las personas que fuimos , prometimos volver porque no queremos convertirnos en una jornada de salud de “vinimos, atendimos, nos fuimos y nos perdimos”, sino que queremos convertirnos en un proyecto concreto continuo, no para reemplazar al estado en sus obligaciones, pero sí para dar soluciones en el momento actual donde hay tanta incertidumbre.
Diagnosticamos muchos pacientes para intervenir quirúrgicamente en las especialidades de cirugía general , mastología, cirugía plástica, ginecología y cirugía pediátrica...
Dejamos órdenes de remisión para todos con sus correspondientes órdenes de exámenes para sus EPS y hoy casi 6 meses después ninguno de los pacientes ha salido de la región porque a pesar de que el derecho a la salud a través de la EPS esté garantizado, desplazarse, además de los costos enormes en transporte y hospedajes, está el temor a abandonar sus tierras y enfrentarse a la violencia de las ciudades y por eso ninguno ha salido, ninguno ha sido operado…
No nos hemos quedado quietos en este tiempo, hemos conseguido organizar un equipo, darle una visión que se acomode a la situación no solo de la Macarena sino de otros municipios que hemos visitado desde Arauca, Vichada Guaviare Caquetá y Guainía, donde hay hospitales básicos con carencias que podemos resolver de manera transitoria para realizar procedimientos quirúrgicos por ejemplo. Hoy no es fácil conseguir recursos, muchos los están buscando para otros proyectos también de carácter humanitario.
Hace años con nuestro primer proyecto logramos convencer a un profesor italiano que intermedió con su gobierno y juntos conseguimos los recursos para construir un hospital de 100 camas, un centro de enfermedades tropicales, puestos de salud, escuelas y granjas agropecuarias con aportes que sobrepasaron los 9 millones de euros. Solo bastó nuestra pasión, nuestro carisma y nuestro deseo de ayudar, así como el apoyo de la comunidad para conseguir esos recursos. Nunca hacemos nada sin ese apoyo. Tuvimos dificultades, pérdidas humanas y enfermedades, pero con el equipo maravilloso que conformamos entonces, similar al de hoy, logramos terminar ese hermoso proyecto en las selvas de Arauca que aún está funcionando.
Y no se perdió un solo euro. Recuerdo nuestros viajes a Italia llevando cajas con recibos, por transporte en la lancha, un almuerzo, un medicamento, hasta los materiales y todo lo demás que se necesitó. Allá nos pidieron que lleváramos consolidados todos los recibos , pero nunca lo hicimos porque mantuvimos la filosofía que eran fondos del pueblo de Italia para el pueblo de Arauca.
Desde entonces hemos hecho otros proyectos y siempre la filosofía ha sido la misma, donde hay una necesidad está nuestro corazón dispuesto a ayudar … solo necesitamos las necesidades de la población para mantener nuestra motivación.
Hoy nos hemos involucrado en la formación de médicos rurales, convencidos que hay que mejorar su consciencia social llevándolos a descubrir ese inmenso país al cual muchos ya no quieren ir por considerar “una pérdida de tiempo realizar el año rural” y nos está yendo bien, necesitamos que vean la importancia que tienen como médicos en el diagnóstico temprano de muchas enfermedades que los especialistas tratamos pero que ellos descubren a tiempo.
Así con los apoyos que tenemos ahora, sobre todo de las fuerzas militares, conmovidas en cómo apoyar socialmente iniciativas como esta para llegar a sitios donde el estado aún no ha podido llegar por diversas razones, es que hoy nos lanzamos a la búsqueda de los recursos que nos faltan.
Los más costosos que son los honorarios que cobrarían los diversos especialistas están asegurados porque somos un equipo de voluntarios que queremos comprometernos con nuestro país, más ahora que somos conscientes que no importa qué sistema de salud esté funcionando siempre habrá espacio para que vayamos a realizar actividades en esos municipios donde de otra manera será muy difícil que lleguen especialistas. Nuestro objetivo nunca será reemplazar el estado pero sí colaborar para que esas poblaciones no tengan que desplazarse a recibir atención en sitios distantes.
Hace 2 años estuvimos en la Pradera, Vichada, normalmente en invierno son varios días para llegar a Villavicencio a recibir atención y como este sitio hemos visitado muchos más como ya lo anotamos, así que tenemos toda la experiencia el conocimiento y la decisión para este compromiso.
Antes de escribir estas líneas comenzamos a describir el proyecto como se escriben estos normalmente, estadísticas, descripción de actividades, descripciones geográficas y muchos más datos... Pero no continuamos, porque esto es un proyecto nacido del corazón no solo del autor sino de nuestro equipo y de las comunidades que nos motivaron a regresar.
Somos un equipo de profesionales que podemos desplazarnos hoy Macarena mañana a otros municipios a realizar actividades médico-quirúrgicas. Somos un equipo de profesionales especializados curtidos en trabajar en sitios con dificultades durante muchos años , conocemos bien nuestro país hemos visitado experiencias en otros países como en África. En lo personal llevo varios años dedicado a implementar un proyecto de diagnóstico temprano del cáncer de seno pasado en el empoderamiento de las mujeres y cómo cuidar sus senos Y cómo a través del auto examen descubrir una lesión sospechosa mucho antes que los signos de alarma clásicos que hoy se enseñan como la panacea para el diagnóstico del cáncer de seno a tiempo ya que todos ellos se refieren a cáncer avanzado.. por eso también puedo aportar esa experiencia de haber visitado más de 100 municipios en nuestro país reafirmando el conocimiento que tenemos de las dificultades que las comunidades tienen para desplazarse a otros sitios.
Así con esta pequeña introducción podemos describir sencillamente nuestro proyecto.
Somos una unidad médico-quirúrgica que puede desplazarse a sitios donde hayan hospitales con unos mínimos requisitos para realizar procedimientos quirúrgicos hasta un cierto nivel de complejidad, basados en que muchos de ellos puedan realizarse con anestesias locales o regionales asistidas.
Sin embargo, requerimos de un apoyo científico y tecnológico de anestesia y para esto necesitamos adquirir una máquina de anestesia portátil sencilla las cuales no pesan más de 12 a 14 kg y funcionan con un vaporizador de ethrane.
Necesitamos un ecógrafo portátil cuya tecnología actual se resume en un equipo muy pequeño portátil, resistente a los movimientos y conectado a una tableta con un programa de inteligencia artificial que nos permite hacer diagnósticos en todos los sitios a donde vayamos. Con esta tecnología a nivel del seno podemos resolver muchos hallazgos clínicos para decidir un diagnóstico presuntivo que nos oriente qué debemos hacer...
Es un equipo multiuso que no serviría para diagnosticar muchas más patologías que como en nuestro primer viaje a la Macarena no pudimos resolver por falta de un diagnóstico más preciso, que un ecógrafo nos podría orientar.
Si bien es cierto en la Macarena podremos encontrar una base de instrumental quirúrgico queremos tener una base propia ya que con los dos equipos anteriores y nuestro propio instrumental podemos ampliar los desplazamientos a muchas más partes en un futuro.
Ya estamos en el camino de concretar otros suministros que se necesitan para iniciar esta labor.
Dentro de los planes futuros está el poder apoyar con este tipo de equipos a la brigada de atención y prevención de desastres ya que esta como ninguna otra más en el país cuenta con elementos para atención médico-quirúrgica en primera línea del desastre, lo cual complementa el desarrollo de un hospital de campaña desplazable a los sitios donde exista un desastre (inundación, avalanchas, terremotos, incendios forestales, erupciones volcánicas, tragedias frecuentes en nuestro país)

HOW TO HELP CARRY OUT SURGICAL INTERVENTIONS IN RURAL AREAS FAR AWAY FROM OUR COUNTRY

In November 2022 we were on a humanitarian mission in the municipality of La Macarena supported by the Omega group of our military forces. Without them we would not have been able to have the knowledge that we have today of the needs of that municipality, since traveling there is not easy...
We travel with our group of professionals with a primary objective. Getting to know the population, asking each person about their illnesses, their difficulties, their desires, their life itself, and thus motivate our hearts about what a project to support that municipality could be. We achieved our objective because we met people who came looking for us from very distant places, we attended more than 400 people and although it is true that we solved many things, we also had the bitter taste of not being able to solve others. And when the president of the community action boards, at the end of the day, introduced himself and offered us the support of the 350 community action boards in the Macarena area, we ended up even more moved to cooperate with these people. We finished the day and with the happiness embodied in all the people we were, we promised to return because we do not want to become a health day of "we came, we attended, we left and we got lost", but rather we want to become a continuous concrete project, not to replace the state in its obligations, but to provide solutions at the present time where there is so much uncertainty.
We diagnose many patients to intervene surgically in the specialties of general surgery, mastology, plastic surgery, gynecology and pediatric surgery...
We left referral orders for everyone with their corresponding examination orders for their EPS and today, almost 6 months later, none of the patients have left the region because despite the fact that the right to health through the EPS is guaranteed, moving, In addition to the enormous costs of transportation and lodging, there is the fear of abandoning their lands and facing the violence of the cities and that is why no one has left, no one has been operated on...
We have not stood still in this time, we have managed to organize a team, give it a vision that fits the situation not only in Macarena but also in other municipalities that we have visited from Arauca, Vichada, Guaviare, Caquetá and Guainía, where there are basic hospitals with deficiencies that we can solve temporarily to perform surgical procedures, for example. Today it is not easy to get resources, many are looking for them for other projects also of a humanitarian nature.
Years ago with our first project we managed to convince an Italian professor who mediated with his government and together we obtained the resources to build a 100-bed hospital, a center for tropical diseases, health posts, schools and agricultural farms with contributions that exceeded 9 millions of euros. Our passion, our charisma and our desire to help, as well as the support of the community, were enough to get these resources. We never do anything without that support. We had difficulties, human losses and illnesses, but with the wonderful team that we formed then, similar to today, we managed to finish this beautiful project in the jungles of Arauca that is still working.
And not a single euro was lost. I remember our trips to Italy carrying boxes with receipts, for transport on the boat, a lunch, a medicine, even the materials and everything else that was needed. There they asked us to consolidate all the receipts, but we never did it because we maintained the philosophy that they were funds from the people of Italy for the people of Arauca.
Since then we have done other projects and the philosophy has always been the same, where there is a need, our hearts are willing to help... we only need the needs of the population to maintain our motivation.
Today we have become involved in the training of rural doctors, convinced that their social conscience must be improved by leading them to discover that immense country to which many no longer want to go because they consider it "a waste of time to carry out the rural year" and we are doing well. We need them to see the importance that they have as doctors in the early diagnosis of many diseases that specialists treat but that they discover on time.
Thus, with the support we have now, especially from the military, moved by how to socially support initiatives like this to reach places where the state has not yet been able to reach for various reasons, is that today we launch ourselves in search of resources that we are missing
The most expensive, which are the fees that the various specialists would charge, are insured because we are a team of volunteers who want to commit to our country, even more so now that we are aware that no matter what health system is operating, there will always be space for us to carry out activities. in those municipalities where it would otherwise be very difficult for specialists to arrive. Our objective will never be to replace the state, but rather to collaborate so that these populations do not have to travel to receive care in distant places.
2 years ago we were in La Pradera, Vichada, normally in winter it takes several days to get to Villavicencio to receive attention and since this site we have visited many more as we have already noted, so we have all the experience, knowledge and decision for this commitment .
Before writing these lines, we began to describe the project as these are normally written, statistics, description of activities, geographical descriptions and many more data... But we do not continue, because this is a project born from the heart not only of the author but of our team and communities that motivated us to return.
We are a team of professionals who can travel today Macarena tomorrow to other municipalities to carry out medical-surgical activities. We are a team of specialized professionals experienced in working in places with difficulties for many years, we know our country well, we have visited experiences in other countries such as Africa. Personally, I have spent several years dedicated to implementing a project for early diagnosis of past breast cancer in the empowerment of women and how to take care of their breasts And how, through self-examination, to discover a suspicious lesion long before the classic alarm signs that Today they are taught as the panacea for diagnosing breast cancer on time since they all refer to advanced cancer. That is why I can also contribute that experience of having visited more than 100 municipalities in our country, reaffirming the knowledge we have of the difficulties that communities have to move to other places.
So with this little introduction we can easily describe our project.
We are a medical-surgical unit that can travel to places where there are hospitals with minimal requirements to perform surgical procedures up to a certain level of complexity, based on the fact that many of them can be performed with assisted local or regional anesthesia.
However, we require scientific and technological anesthesia support and for this we need to acquire a simple portable anesthesia machine which does not weigh more than 12 to 14 kg and works with an ethrane vaporizer.
We need a portable ultrasound machine whose current technology is summarized in a very small portable equipment, resistant to movements and connected to a tablet with an artificial intelligence program that allows us to make diagnoses everywhere we go. With this technology at the breast level we can resolve many clinical findings to decide a presumptive diagnosis that guides us what we should do...
It is a multipurpose piece of equipment that would not be used to diagnose many more pathologies than on our first trip to La Macarena we could not solve for lack of a more precise diagnosis, which an ultrasound machine could guide us.
Although it is true that in La Macarena we will be able to find a base of surgical instruments, we want to have our own base since with the two previous teams and our own instruments we can expand the displacements to many more places in the future.
We are already on the way to specifying other supplies that are needed to start this work.
One of the future plans is to be able to support the disaster care and prevention brigade with this type of equipment, since this, like no other in the country, has elements for medical-surgical care on the front line of the disaster, which complements the development of a mobile field hospital to places where there is a disaster (floods, avalanches, earthquakes, forest fires, volcanic eruptions, frequent tragedies in our country)
